Please print and fill out the new patient paperwork (see below), and bring it in with you or show up 20-30 minutes early to complete it prior to your appointment.
Ensure that you bring your photo ID and applicable insurance cards.
Any applicable co-payments or time of service payments will also be collected prior to check-in.
It is recommended to wear comfortable clothing.
Your first appointment with the chiropractor will include an initial exam, where you can discuss your concerns and your health history. The initial exam will help your chiropractor determine the best possible treatment options to help you reach your healthcare goals. Your first appointment includes an adjustment, which will be discussed prior to treatment. At the end of your appointment, the chiropractor will go over your personal treatment plan, which may include additional visits, other types of treatment, such as massage or even some at home stretching.
